General Policies
How we do things

For non-urgent questions please contact us via the Patient Portal or call during our office hours when we are in clinic. You can reach us at (971) 231-HLFM (4536).

If you have an urgent health issue and require immediate assistance or advice from Dr. Kerry please call (971) 231-HLFM (4536). If she doesn’t answer right away, please leave a message and she will get back to you as soon as she can. Please use this privilege appropriately as it means interrupting work or family time.

For a life-threatening emergency, always call 911 immediately.

There may be rare times when Dr. Kerry will not be able to return your urgent call promptly (such as the cell phone battery dying unexpectedly) and what was an urgent situation becomes an emergent situation. In these cases, please don’t delay in seeking emergent care or calling 911.

Email can be a convenient way for us to communicate, however I encourage you to use our patient portal for most questions. Until you are set up with a portal account you can send me an email .

Please be aware that email is not entirely secure. For example, if your email address is through your employer, your employer may be able to read the messages. Likewise, there is a small possibility of interception of messages from outside parties (“hackers”). For these reasons, it is best NOT to transmit messages of a sensitive nature, such as mental health, substance abuse, or HIV status through standard email.

Your appointment slot is long (typically 30-60 minutes depending on appointment type). Although we may not use the entire time slot for you appointment, this time is reserved for you and no one else. Please cancel your appointment online at least 24 hours prior to your appointment if you need to cancel. This will allow time for the slot to be filled by another patient. If you do not notify me at least 24 hours beforehand, you will be charged a $50 cancellation/no show fee.

If you do not come to your appointment without cancelling (“no show”), or cancel with less than 24 hours notice, you will be required to provide a credit card number in order to schedule another appointment. On the second “no show” or less than 24 hour cancellation, the missed appointment fee will be collected.

Refilling prescriptions consumes a surprisingly large portion of staff time in a standard medical office. As we will be operating this clinic with minimal staff, we will not be able to refill prescriptions outside of office visits. When Dr. Kerry writes a prescription, she will give you enough refills to last at least until your next scheduled office visit. If your refills are running low, it likely means it is time to schedule your follow up appointment!

For medications that arenʼt used every day of the year, like allergy or headache medications, this policy will require you to be mindful and anticipate future refill needs during your current appointment.

If all else fails and you end up needing an emergency refill outside of an office visit, send a message or call with the drug name and dosage as well as the pharmacy phone number. We will attend to it within 24 hours. You will be charged a $15 administrative fee for this service for the first prescription, $5 for each additional prescription.

Please do NOT have your pharmacy contact us for refills. These refill requests are often auto-generated at the pharmacy or are simply incorrect, thus we will ignore them. You must contact us personally if you need an emergency refill outside of an office visit.

Dr. Kerry rarely believes it is in the best interest of her patients to rely on chronic narcotics or sedatives, and thus infrequently prescribes them. If you come to her on chronic narcotics or sedatives she will work to decrease your dose to the lowest tolerable level. Additionally, for both of your safety and hers, she requires a signed contract and random drug testing for continued prescribing.

By law, these medications cannot be refilled over the phone.

We hope you will never need to be hospitalized. If hospitalization does have to occur, Dr. Kerry feels that you will be best served by the hospitalist physicians (specialists in the care of hospitalized patients). Dr. Kerry will visit you at the hospital if you would like, and will communicate closely with your in hospital doctors about your health issues and the outcomes of your hospitalization.

Please tell the hospital that Dr. Kerry is your primary care physician so they can update her with all important information. Also, within a day or two of your discharge, please send a message or call so we can determine how soon you need to be seen for follow-up.

There will be times when Dr. Kerry will be out of town or otherwise unreachable. Dates of upcoming vacation plans will be posted on the home page. If you have an urgent health question or need to be seen while she’s away please check the home page as it will also include a link to the website of her colleagues who can help you in her absence.

Dr. Kerry is not currently accepting new patients except for family members of current patients. If you have been accepted as a new patient, please see the FAQ What should I do if I am a new patient? for further information.

On July 1st 2019 we are excited to integrate a new model of care into our practice, with the goal to allow more people access to necessary health care services.

In place of our annual administration fee, we are now implementing a sliding scale annual membership paid in monthly installments. With this membership payment, you are sustaining all the wonderful services Healthy Living Family Medicine is able to offer to patients at all income levels.

You may terminate our relationship at any time for any reason. We would appreciate communication from you expressing your desire for termination, but it is not required.

Likewise, Dr. Kerry may terminate your relationship at any time. Generally, she will reserve this measure for patients who are not abiding by the stated policies, are delinquent in paying bills, or are disrespectful to her, her clinic or her staff. If she chooses to terminate the relationship, she will notify you in writing. You will then have 30 days to find a new physician, during which time she will be available to you for urgent health issues only. You may request a transfer of medical records to your new physician.

How our clinic works

About the New Clinic Model

In an ideal world, health insurance would increase the accessibility of health care. It would truly be an investment in people’s health. Unfortunately, I often hear from patients that this isn’t true. Increasingly, people are paying more per month, with higher deductibles and fewer benefits. This discourages people from getting the care they need.

Additionally, several insurance companies have determined that because I am not considered a “Medical Home”, I am not eligible to be in the network for many of their plans. This means that patients either pay full price to stay with their preferred provider, or are forced to see new providers who don’t know their history and with whom they don’t have a relationship.

It is important to me to find a way to support my patients outside of the control of the insurance companies. I aim to create a model of primary care that will allow me to serve my patients in a way that is affordable and sustainable. My goal is that this new model will allow patients to feel that they are able to make a true and more meaningful investment in their health.

Sliding Scale Monthly Membership Payments

In place of the annual administration fee, we are pleased to offer sliding scale monthly payments for health care. Each individual, or family, is responsible for determining their own monthly payment capabilities given their current financial situation. This model is designed to allow more people access to necessary health care services and to maintain the unique, holistic care you receive at Healthy Living Family Medicine.

Membership Fee - What you get

With your membership fee you are getting and sustaining:

  • Our calm, welcoming clinic environment.
  • Long appointment times that prioritize your health needs.
  • 24/7 access to Dr. Kerry via phone or portal messaging.
  • Reduced cost for the care resources: supplements, essential oils, etc. that we love to provide for you.

Membership Fee - Amounts

Our sliding-scale for the monthly membership fee is $5 - $35/patient/month.

Membership Fee - Policies

  • Individuals or families who have already paid their annual administration fee for 2019 are covered for this year and will begin monthly payments in January 2020.
  • Families can combine payments for all family members into one monthly membership cost.
  • You may pay your membership fee as a yearly lump sum rather than a monthly fee if you’d prefer.
  • In November of each year all patients will be asked to sign up again and re-commit to a membership fee level for the following year.

Pay by Appointment

When you or a family member comes in for an appointment, you will be asked to pay what you can for the visit at that time. Though Dr. Kerry feels uncomfortable assigning monetary value to her time, we ask that you please consider the time of other clinic staff, the rental of space, cost for blood draws or procedures, as well as the unique, holistic, care that you recieve at this practice. We are committed to being transparent regarding the true cost of our services. See the accompanying page for more information on our cost for services.

Determining Payment Capability

Factors to consider when determining payment capability.

Consider paying the maximum monthly, or full appointment cost, payment amount if you:

  • own the home you live in.
  • have a stable job.
  • work part-time or choose to stay at home with your children by choice, not necessity.
  • have investments, retirement savings, or inheritance.
  • have resources available (family, savings) that you could access in a time of need.
  • have insurance that covers a majority of your health care services.
  • have an educational background that allows you to attain higher earnings, even if you are not currently employed.

Consider paying a reduced monthly or appointment payment if you:

  • have significant debt.
  • have medical expenses that are not covered by your insurince.
  • are uninsured.
  • financially support parent(s) or dependent(s).
  • are a student.
  • are unemployed, or have been recently laid off.
  • receive state or federal assistance.

Additional Payment Considerations

  • Individuals who choose to pay for a service (procedure or vaccination) in full will have the opportunity to request a superbill. This superbill will allow for the possibility of insurance reimbursement for all services you receive at this clinic. Please see this page for our standard full cost for services.
  • We are happy to offer alternative, non-monetary options to pay for an appointment, such as barter. Dr. Kerry will come to an individualized agreement on barter terms with one who chooses this option. Communicate with Dr. Kerry if this sounds like the best option for you given your financial situation.

Patient Care Fund

Healthy Living Family Medicine is proud to establish a Patient Care Fund. Individuals can donate to this fund to directly support health service costs for those who may be unable to afford payments at the time. 100% of this donation will go towards patient care.
