Questions about our practice

Healthy Living Family Medicine – Primary Care is not taking new patients at this time.

If you are a family member of a current patient, Dr. Kerry may be able to see you. Please contact us at (971) 231-4536 to find our more.

For non-urgent questions please contact us via the Patient Portal, or give us a call during our office hours when we are in clinic. You can reach us at (971) 231-HLFM (4536).

If you have an urgent health issue and require immediate assistance or advice from Dr. Kerry please call (971) 231-HLFM (4536). If she doesn’t answer right away, please leave a message and she will get back to you as soon as she can. Please use this privilege appropriately as it means interrupting work or family time.

For a life-threatening emergency, always call 911 immediately.

There may be rare times when Dr. Kerry will not be able to return your urgent call promptly (such as the cell phone battery dying unexpectedly) and what was an urgent situation becomes an emergent situation. In these cases, please don’t delay in seeking emergent care or calling 911.

Dr. Kerry is available for patient appointments during the following hours:

Pearl will be available to address phone and portal communication and walk-in needs during these times:

Call us to reschedule soon as you know that you won't be able to make an appointment. If you need to cancel, please call at least 24 hours prior to your appointment to avoid a $50 cancellation / no show fee.

In place of our annual administration fee, we are now implementing a sliding scale annual membership paid in monthly installments. See Will I still pay the annual admin membership fee? for additional details.

Welcome to Healthy Living Family Medicine.

We will set you up with a Portal Account, then send you a link to the new patient questionnaire to fill out prior to your first appointment.

Please bring the following to your first appointment:

  1. ID
  2. Insurance information (if applicable)
  3. Your medications and supplements (in their packaging)
  4. New patient paperwork
  5. Immunization record (for both adults and children

Please review or fill out these forms as appropriate.

Privacy Practices Form -- Please review this form. We will ask you to sign a form noting that you reviewed it when you come in for your appointment.

Finally, please review our Office Policies in detail. At the office we will ask you to sign a form noting that you have reviewed this and agree with our policies.

When Dr. Kerry writes a prescription, she will give you enough refills to last at least until your next scheduled office visit. If your refills are running low, it likely means it is time to schedule your follow up appointment! For medications that arenʼt used every day of the year, like allergy or headache medications, this policy will require you to be mindful and anticipate future refill needs during your current appointment.

Healthy Living Family Medicine provides a Patient Portal for Dr. Kerry's patients.

The Patient Portal provides convenient round-the-clock access to your healthcare information. See the date and time of upcoming appointments, schedule a new appointment, or send Dr. Kerry an email. View your bills and pay any outstanding balances. Review the clinical summary of your past appointments and access your Plan of Care.

Dr. Kerry does recommend and sell some supplements. These are sold to you both in her office and on the Wellevate link. She provides these at her office, since it is difficult to find well-regulated and reliable supplements at local stores. You can also visit Dr. Kerry's Supplements page to find approved options:

Questions about our clinic model

Why are you choosing to practice medicine without taking insurance?

In an ideal world, health insurance would be making health care more accessible to people. It would truly be an investment in people’s health. Unfortunately I frequently hear stories from patients how this isn’t true. Increasingly people are paying more monthly, with higher deductibles and fewer services covered. This leads people to avoid getting the care they need.

Additionally, several insurance companies have limited the providers allowed to be in the network for many of their plans. This means that patients are either paying full price to continue to see the provider they most want to see, or are seeing new providers who don’t know their history and with whom they don’t have a relationship.

It is important to me to find a way to help support my patients outside of the control of the insurance companies. My hope is to create a model of primary care that will provide good care to my patients in a way that is affordable and sustainable. My goal is that through this new model patients will actually feel that they are able to make an investment in their health.

What are the benefits of this new model?

  • You won’t receive bills for services the insurance companies choose not to cover.
  • You will know how much you are spending on your health care and not wondering how much your insurance plan will pay.
  • Continued access to the comprehensive care that Kerry provides.
  • If your insurance plan has to change due to work or the Marketplace you will be able to have continued support and continuity with Dr. Kerry.

Will I still be able to use my insurance for referrals to other providers, lab work and imaging?

Yes, individuals with insurance will still receive their standard insurance coverage for all lab testing, imaging, prescriptions and referrals to other providers. We will collect insurance information to send along with lab and imaging orders and referrals. Labs, imaging centers and specialists will bill your insurance directly.

Will I still pay the annual admin membership fee?

All patients will be asked to pay a new membership fee in place of the annual admin fee. Unless you prefer paying annually, the fee will be paid via monthly deductions from your credit card or bank account. Bank account withdrawal is preferred to help keep our costs down.

How much will the monthly membership cost?

The membership fee will be on a sliding scale of $5 - $35/month. Please consider paying a higher monthly fee if you frequently contact me for advice via the patient portal and/or with phone calls. You may also add a donation amount to your monthly deduction as a contribution to our Patient Care Fund (see What is the Patient Care Fund?).

When do membership payments start?

If you have already paid your annual membership admin fee for the 2019 year, you are covered for the rest of the year and will not have to start making membership payments until January, 2020. Though, you may choose to add a monthly or one time donation to the Patient Care Fund. Membership payments agreements will be re-submitted in November of each year.

If you have not paid your annual membership fee for the 2019 year, monthly membership payments start when the new clinic model is implemented on July 1st, 2019.

What services am I paying for with my monthly payment?

  • Access to the unique, holistic and nurturing care given by Dr. Kerry and Pearl.
  • Our calm, welcoming clinic environment.
  • Long appointment times that prioritize your health needs.
  • 24/7 access to Dr. Kerry through phone and patient portal.
  • Reduced cost for care resources: supplements, essential oils, etc.
  • Access to the new in-clinic sauna.

How much can I expect to pay for appointments?

It is expected that at each appointment patients will pay what they are able to and what they feel is appropriate given the unique, holistic and nurturing care they receive from Dr. Kerry and Pearl.

This is a sliding-scale model that ranges from paying full cost for a visit to whatever your financial situation allows.

Which (sliding scale level) is the right one for me?

When paying for an appointment, please consider your (or you and your family’s) current financial situation (including housing, employment, debt and investments) as well as the holistic, patient-centered, care you receive at this practice.

Additionally, when determining your monthly membership payment level, please consider paying a higher fee if you frequently contact me for advice via the patient portal and/or with phone calls.

Above all, please do not stress about this process. Pay what feels right for you or your family at this time.

How much is the full cost for an appointment?

We are committed to being transparent regarding the true cost of our services. Having averaged, Dr. Kerry’s time, clinic staff’s time, rental of clinic space, and admin costs, we have calculated our cost for a one hour appointment to be $240. See the “Services Price List” for more information on our true costs.

May I request a superbill so I can submit it to my insurance?

Yes, you will have the option to request a superbill. You may then take the superbill and submit it to your insurance for reimbursement.

Can I use my FSA/HSA to pay for appointments?

What is the Patient Care Fund?

Healthy Living Family Medicine is proud to establish a Patient Care Fund. Individuals can donate to this fund to directly support health service costs for those who may be unable to afford payments. 100% of this donation will go towards patient care. We are working towards acquiring a non-profit status for the clinic. Currently donations to the Patient Care Fund are not tax-deductible. We appreciate your generosity in helping others receive unique, holistic, patient-centered care with Dr. Kerry.
