Payment options

We accept several insurance plans as well as non-insurance options.

Dr. Kerry is currently a participating provider with the following insurances:

  • FamilyCare
  • Blue Cross/BlueShield
  • Providence (Open or Choice) – but NOT “Medical Home”
  • Pacific Source
  • Aetna
  • Medicare
  • United Health Care
  • ODS
  • Lifewise
  • Cigna
  • TriCare

She will NOT be able to see CareOregon or HealthNet patients at this time.  If you have CareOregon please contact your case manager about switching to FamilyCare or Open Card so she can see you.

She does see worker’s comp and auto insurance for all established patients.  She will see some new worker’s comp/auto insurance patients on a case-by-case basis.

If you are a member of one of these insurance plans, please bring your member card to all appointments and notify us of any changes to your insurance status. We will submit bills to your insurance for you. You will need to pay any co-pays at the time of your visit.

Payment may be made by cash, check or credit/debit card.  We prefer cash or check.  You will be responsible for any balances your insurance deems are your responsibility (such as a deductible or co-insurance) and for full payment of non-covered services. Each insurance plan differs in what services it covers. For example, not all insurances cover physical exams or certain vaccinations.  It is your responsibility to understand your insurance and what is covered.

If you have insurance but your plan is not listed above, we will prepare a bill for you to submit to your insurance for reimbursement. You will need to pay the office charges in full on the day of service.

If you are without insurance you have a few options:

  • Receive a 30% discount by paying in full on the day of your appointment.
  • Connect to Dr. Kerry through the Patient/Physician Cooperative and pay a monthly fee to get five appointments per year.  You can see details at:

Please call if you have specific questions about my rates.

My billing service will notify you if you have any outstanding charges.  If you are suffering financial hardship, please contact them so we can negotiate a payment plan with waived late fees.

A fee of $25 will be imposed for a bounced check.
